Do people have a right to form a government if they so choose?

1. If 200 shipwrecked people live on a deserted island, do they have a right to be anarchists if they so choose?
2. Do they have a right to reach a consensus on who owns (or gets to live on) what part of the island?
3. Do they have a right to set and to enforce a code of conduct?
4. Do they have a right to form a government if they so choose?

They do have that right
Depends on the circumstances....If they take it by force then that right is not going to last.
Discuss the whole situation and the pros & cons of the island....Divide it between the people equally. You now have the right to form a government.
Or lets say a post civil war country that was devastated...you are part of the force that's left. As long as you make decisions with the other people left behind, you would be able to sustain a balanced government.
Make the wrong choice....and the deck collapses. We see it in all official nations.

we live under maritime/admiralty law - NOT the Constitution
What most people around here don't understand is their unlimited right to contract, or not to contract
everyone's in such a fuss about contracts they feel they are compelled to adhere to - when they could actually simply remove themselves from or adjust at their will

positive vs. negative rights
Your list consists entirely of positive rights.
Positive rights describe what one is allowed to do. Negative rights describe the right to be free from force.
Positive rights only exist when one is dealing with one's own property or when others give consent.
The answer to all your questions, except #2 is YES, because each person has given consent and made an agreement.
The answer to #2 is NO, because, as I stated above, positive rights only exist with one's property or when others give consent. There is consent, but it's not their property. They have not homesteaded the entire island. No one owns any portion of the island until they have homsteaded it.
If someone new visits the island, that person is free from any government coercion created by the original 200 people, provided that the new person is not trespassing the homesteaded portions of the island.
If one of the shipwrecked people sells a portion of their property to the new person, that new person is still free from their government (unless the purchase agreement says otherwise).
Positive and Negative rights
And after they form a government....
... and the first baby is born....
What then? Is that baby held to the code of conduct before it reaches adulthood? And if the baby reaches adulthood and does not consent to that code of conduct, can he be rightfully detained, sentenced, or punished?

When he is an adult, he is like a newcomer to the island. He must follow the rules when on other people's property.
On his own property or on non-homesteaded places of the island, he does not have any positive obligations and does not need to follow the rules (except for respecting everyone's negative right to be free from force).

to start a revolution if it dose not like the rules of the island or he can build a raft and leave the island. If someone wanders onto the island they can do the same thing. "The tree of liberty must be refreshed from time to time with the blood of patriots and tyrants." Its only a fool that would think that man can live without any agreed on rules of law and just treat others in a fair manner because its the right thing to do. Government should be formed to protect the rights of individuals and when it fails to do that then a new government should be formed.
Lets also not forget a huge issue the anarchist fail to ever address and why the Constitution was created, and that is, some kind of government is needed not only to protect the rights of the people living within the state but also to protect the people from foreign governments and invasion. Their is no super nova type of event that will ever occur where every nation in the wold will all at once embrace the anarchist philosophy. At your boarder will always be a nation that will invade you and take over your land because you have no means to protect your self.
You have a choice pick the form of government you want or just let someone else force it on you.
