20 years seems like a

20 years seems like a reasonable time to study the effect of a drug on patients health outcomes when it is involving something rare and slow developing like cancer. A study would need to enroll hundreds of individuals as control or test group participants, do their best to balance variability and ensure each group is faithful to their label. Then wait to see the incidence of cancer in each group over time.

I don't doubt they will abuse the system to milk extra dollars out of everyone; but I don't think 20 years is really all that slow for researchers.
