T-mobile "pay as you go" plan. $100 for 1,000 minutes that don't expire for a year, and then if you haven't used them up you can just put another $10 on it and roll the minutes over for another year. If 1,000 minutes is about what you use in a year that's $100 for minutes and $50 for the phone = $150/12 = $12.50 per month. Less if you look around for a compatible used phone.
If you buy it in person you can pay cash get it without ever having your name or credit card number associated with it, in case that matters. I've got one that I paid $30 for four years ago, and it's been my primary phone for most of that time and more recently just a backup phone, for $100 the first year and $10 each year after that.
